Referral Information

Referral under "Medicare" requires the patient's General Practioner to complete a , Mental Health Care Plan (Item no. 2710). Information requested from the referring GP include:

1. Contact details of referring Medical practitioner (including GP's Medicare Provider details)
2. Relevant Mental Health issues for treatment
3. A copy of the Mental Health Care Plan
4. Contact details of the client

Legal Referral

Referrals for psycho-legal matters are best made by the client's legal practitioner.

In this referral it is recommended that Legal Practitioner at the time of making the referral appointment provide the following information:

1. Contact details of Legal practitioner
2. Relevant legal issues for examination
3. Funding details and funding agent
4. Date when Report is requested
5. Contact details of the client

General Psychological Referral

Referrals for Mental Health assessments (not Medicare funded), forensic assessments (not psycho-legal funded) vocational assessments (not Workcover funded) and Sport Psychology Programs can be arranged by the individual or Organisation (General referral) through a direct referral to the Practice Manager.
At the time of making the referral appointment ensure the following information is provided:

1. Contact details of client
2. Relevant issues for examination
3. Funding details and funding agent
